Subject-verb singular plural agreement:

The singular subject singular verb, plural subject plural verb agreement is one of the main grammatical errors which could occur in Medical Transcription documents. Below are a few examples:

  1. The patient say he is intolerant to caffeine substances (Wrong). The patient says he is intolerant to caffeine substances (Right).
  2. The patient were prepped and draped in sterile manner (Wrong). The patient was prepped and draped in sterile manner (Right).

Punctuation errors:

Using of proper punctuation is extremely important for Medical Transcription. Below are a few examples:

  1. Father died of kidney cancer, mother, breast cancer. (Wrong) Father died of kidney cancer; mother breast cancer. (Right)
  2. His medications are gabapentin Singulair, Prevacid. (Wrong) His medications are gabapentin, Singulair, Prevacid. (Right)

Tense errors

Using correct tense is extremely important for Medical Transcription. Below are a few examples:

  1. The patient was asked to following up with me in 10 days. (Wrong) The patient was asked to follow up with me in 10 days. (Right)
  2. This was a 66-year-old woman who is complaining of GERD (Wrong) This is a 66-year-old woman who is complaining of GERD. (Right)

Preposition errors

Using correct preposition is extremely important for Medical Transcription. Below are a few examples:

  1. The patient was seen in 10/18/2019. (Wrong) The patient was seen on 10/18/2019. (Right)
  2. The patient lives on Kentucky (Wrong) The patient lives in Kentucky (Right)

Gender errors

Using correct gender is extremely important for Medical Transcription. Below are a few examples:

  1. This pleasant 25-year-old lady was seen by me on 10/18/2019. He was complaining of diarrhea. (Wrong) This pleasant 25-year-old lady was seen by me on 10/18/2019. She was complaining of diarrhea. (Right).
  2. The patient is a 66-year-old man who is complaining of foot pain. She says the pain has been present since one week. (Wrong) The patient is a 66-year-old man who is complaining of foot pain. He says the pain has been present since one week. (Right)

3.  A transcriptionist called an MT (a Medical Transcriptionist) proofs the report with voice file word to word.

4. Transcriptionist then reads the report without listening to the voice file and re-listens words or phrases that may require a double check.

5. An editor called a TE (Transcription editor) proofs the word file with voice.

6. The TE reads the report without listening to the voice file and re-listens words or phrases that may require a double check.

7. We upload the text files securely to Transcription Manager for the client to download. If the client has his/her own platform and specifications, then we follow that.

8. Feedback, if any, from the client is taken and studied for improvement.
